Policies
When you schedule
ETM is available by appointment only.
All you need to reserve your appointment is a phone or computer and a credit card. You may choose to use another form of payment (cash or check) once the services have been rendered. When scheduling, please do not commit to a time if you are uncertain as to whether or not you will be available at the scheduled time. Please refer to the cancellation policy below.
Prior to your massage
Please have your cell phone ringer turned OFF to avoid any disruptions during your session.
Refrain from consuming alcohol at least six hours prior to a massage.
Both therapist and clients are required to follow proper hygiene etiquette.
Any food should be consumed two hours prior to the massage to allow time for digestion.
As much as we love children at ETM, the studio is not designed to keep them entertained.
This is your time!! Please make arrangements for childcare.
Payment Policy
Payment is due at the time of service. Acceptable forms of payment are cash, check, Visa, MasterCard, Discover or American Express. Credit card charges are handled through PayPal. Please be prepared to show a valid driver's license. Returned checks are subject to a $35 fee. There are no refunds once services have been rendered.
ETM does not provide insurance billing services. Please notify your therapist if you intend to submit an invoice or documentation for financial reimbursement. Chart notes and receipts for sessions are available to submit for reimbursement.
Although not expected, gratuities are gratefully accepted at your discretion.
Because your therapist can only work on one client at a time and offer a limited number of massage appointments each day, your therapist asks that you help make the most of the time you have scheduled for your session by honoring the following guidelines:
Arrival Time
Please arrive for your scheduled appointment approximately 15 minutes early to complete the Client Intake Form/Discomfort Diagram, use the restroom and ensure adequate time to discuss with your therapist any changes in your health or medical status.
If you are arriving late, please give a courtesy call. Late arrivals are not granted extensions but will receive the remainder of their allotted time and full session price will be due. It is a courtesy to be timely. (It is not possible to run over scheduled times when other clients are waiting.)
Cancellations/Missed Appointments
Please give at least 24 hours notice if you cannot make your appointment. Cancellations in less than 24 hours result in a $40 cancellation fee. "No shows" will be responsible for the full value of the scheduled appointment. The charges will go on the credit card you gave when scheduling your appointment. These rules also apply to gift certificates. Missing an appointment without calling to cancel causes a hardship for the therapist and someone else who may have been denied that appointment time.
Illness
In the case of an illness please reschedule your massage session. This is both for the health of the therapist and that of others receiving massage in this office. Usually massage can also exacerbate your illness due to the circulation enhancing qualities of massage.
Client Intake Form
Please fill out the Client Intake Form to the best of your knowledge. The more your therapist knows of your medical history, the better she can adjust the techniques used during your massage. If you are unsure how to answer a question, just ask. Please update your form when necessary (e.g., recent surgeries, new medications, injuries, painful areas or areas of concern, etc). Any information you provide is extremely private and confidential.
Intake forms are available for printing to the right of the screen. Please print and fill out prior to your first appointment.
Outcalls
It is the client's responsibility to provide an environment free of distractions, i.e. pets, phones and people. Walkways and stairways to the location must be cleared for easy, unimpeded access. Please scheduled 24 to 48 hours in advance.
Referrals
All referrals are welcome.
Receive 15 minutes of massage for every person you refer that receives a one-hour massage.

Professional Massage
Your therapist is a licensed massage therapist in the State of Alabama. Your therapist cannot diagnose medical diseases or disorders, prescribe medications, or perform manipulations or adjustments of the human skeletal structure. She will be happy to refer you to those who are licensed to do so.
Right of Refusal
ETM reserves the right to refuse service to anyone. This includes, but is not limited to, anyone who requests services that are outside the therapist's scope of practice.
ETM reserves the right to refuse service to anyone that arrives for a session under the influence of alcohol or recreational drugs. Under these circumstances, full payment is expected for the session time, whether or not services were rendered.
ETM provides therapeutic massage and any sexual remarks or advances will immediately terminate the session and full payment is expected for the session time.
To Prevent Any Misunderstandings...
EquiLibrium Therapeutic Massage, llc provides only non-erotic, professional massage. Any act of sexual misconduct or harassment by the client will immediately terminate the session, and full payment will be due. ETM has the right to refuse clients from booking or rescheduling.
